To apply for CPT, you must go through your Designated School Official (DSO). You need to apply online before starting your internship, clerkship, or practicum. The following documents are required for the CPT application:

  • CPT Employment Information (online form)
  • Department Recommendation for CPT (online form)
  • CPT Internship Offer Letter: This must include:
  • Employer letterhead, signed and dated by a supervisor or HR representative.
  • An original offer letter for applying for a Social Security Number (SSN); electronic or copied letters are not accepted by the local SSN office, though the UFIC does not require an original.
  • Employer’s EIN number (required by the local SSN office). The UFIC does not require the EIN.
  • Current date.
  • Student’s given name and family name.
  • Position/job title.
  • Start and end dates of the training/work.
  • Expected number of hours per week for training/work.
  • Exact physical address where the training/work will take place (including street, suite number, city, state, and zip code).
  • General description of duties (3-5 sentences or bullet points). If not included in the offer letter, an addendum must be provided, along with the supervisor’s name, title, and phone number.

After you submit your CPT application online, your International Student Advisor (ISA) will review it. If approved, you will receive a new I-20 with CPT authorization on the second page. You can only begin your training or work after receiving the authorized I-20 from your ISA.
